- Personal details such as your name, age, address,telephone numbers, email address, and your general medical practitioner.

- Your past and current medical and dental condition.

- Radio graphs, clinical photographs, and study models.

- Information about the treatment provided or proposed,along with associated costs.

- Notes of conversations or incidents necessitating record-keeping.

- Records of consent to treatment.

- Correspondence with other healthcare professionals, such as referrals to specialists.


Comprehensive and accurate patient data is crucial for providing safe and appropriate dental care. Our data processing proceduresinvolve:·

- Retaining your dental records while you are a practice patient. If you cease to be a patient, we will continue to hold them for at least another eleven years or until the age of 25 for children, whichever is longer.

- Storing personal data in our practice's secure computer system and/or a manual filing system, accessible only to authorised staff.

- Ensuring secure audit trails in our computer system and daily backups of information.


For proper and safe dental care, we may need to disclose personal information to:

- Your general medical practitioner.

- Hospitals or community dental services.

- Other health professionals involved in your care.

- Dental insurance companies and private dental schemes of which you are a member.


Disclosure occurs on a "need-to-know" basis,ensuring information is shared only with individuals/organizations essential for your care and proper government administration.

In limited circumstances, personal data may bed is closed to a third party when required by law or court order. All other disclosures will only happen with your specific consent.

If you move to another dental practice, we may, at our discretion, provide original X-rays and copy notes to the new practice up on their written request, free of charge.

If you wish to discuss or object to the disclosure or use of your personal data as outlined in this policy, please engage with your dentist. Note that objection may impact our ability to provide dental care.
